Abstract
Background: Chronic diseases are increasing
world wide. The chronic care model is a framework to improve the care for chronic ill patients. The goal of this study is to get a survey of the experience and opinions of general practitioners according to the integration of communal resources, one element of CCM. Method: 14 general practitioners were interviewed. Analysis and categorization were carried out with Atlas ti.Results: General practitioners see resources in prevention but also difficulties in organisation,coordination and/or cooperation. Further on, barriers are the huge amount of time and the lacking motivation of the patients in using communal resources.Conclusion: One possible solution may be a “roundtable” of the stakeholders in the community.
Further studies with certain emphasis on the relevance of community resources within the bio-psycho-social model of health care for patients with chronic conditions are necessary.
